Clinical Manifestation of Vertigo

Benign paroxysmal positional vertigo patients (BBPV) will complain if the head turns on its specific circumstances. Patients will find it rotates or spins around her feel when going to bed, rolling from side to side, get out of bed in the morning, to achieve something high or if the head is moved backwards. Vertigo usually lasts only 5-10 seconds. Sometimes accompanied by nausea and often patients feel anxious. Patients can usually recognize this situation and try to avoid it by not doing movements that can cause vertigo. Vertigo will not happen if the head upright or rotating axially without the extension, in most of the patients, the vertigo will be reduced and eventually stopped spontaneously within a few days or several months, but can sometimes also up to several years. In BPPV did not get a hearing loss. The diagnosis of BPPV is made based on history, clinical symptoms ENT examination, test position and caloric test.

At the anamnesis, the patient complained of head feels dizzy spin on changing head position with a certain position. Clinically vertigo changes occur in head position and will be reduced and eventually stopped spontaneously after some time. On ENT examination found no abnormalities in general means, and the caloric test no canal paresis. Tests can help diagnose BPPV position, the best is to do a Hallpike maneuver: the patient sat upright, his head is held on both sides by the examiner, then suddenly dropped his head as he turned to one side. In this test will get a position with symptoms of nystagmus:
  • Eye rotates and moves toward the affected ear and subside after 5-20 seconds.
  • Accompanied by severe vertigo.
  • At a latency period was preceded symptoms for a few seconds (3-10 seconds).
  • In the repeat test will be reduced, therapy is also useful as a means of diagnosis.
